New Client Intake-Annual Fee-Assessment

Service Description

VIA EMAIL/ONLINE PROCESS (Valid for 1 year since first service received) Nutrition Profile and/or Physical Level assessments. The more i know about you the better advice i can provide. Information collected and reviewed: your health history, nutrition habits, habits in general, your daily challenges, issues, anything you would like to add discuss that is besides on forms to be filled out. There will be questionnaires to fill out. It is imperative to learn a client for me. I learn about you to be able to service you with coaching and/or Fitness Training. This New Client Intake & Yearly Fee is required for any program start up and paid once a year to receive services. Fee valid for 1 year (paperwork update / changes in health history should be communicated by a client, all advice will be based on the information received at the time of paperwork received!). Fee valid 1 year. Example: If first service received 02/07/2023 your paperwork is valid till 02/07/2024.
